1.各位同仁這次公費疫苗登記有意願施打的同仁，請先到人事室來填寫 `流感疫苗入校接種同意書及評估表`~以免當日接種人數眾多而產生許多混亂情況~
2.此次疫苗有分中央公費及桃園市自購之疫苗，而在上次請同仁填表單時，若有在年滿50歲等幾個問題中填 是 之同仁(如附件word檔名單)，是屬於 中央公費 的部分，來填謝同意書時，請一併攜帶對應資格 需要檢附之資料 ~謝謝!!!
3.中央公費word檔名單 `以外` 之同仁 ，是屬於桃園市自購之疫苗 ~僅需來人事室填同意書即可~ ~
有登記要施打之同仁請於 9/30中午前 來人事室填寫~~謝謝'!!!
