說明：
一、依據教育部國民及學前教育署補助辦理中輟生預防追蹤與復學輔導工作原則及桃園市政府教育局中華民國109年4月8日桃教學字第1090030802號函辦理。
二、109學年度如欲申請個案轉介本校資源式中途班就讀，請依附件規範內容、文件格式及期程提出申請轉介，請郵寄掛號至本校輔導室，轉介時間於即日起至109年10月16日止。
三、轉介學生學籍學校提出申請後，經本校復學輔導會議審查通過後，將通知申請學校審查結果。
四、檢附本校中途班實施計畫、同意書及轉介就讀申請表。
