| 一、 | 依據嘉義縣政府110年2月19日府教學特字第1100037497號函辦理。 |
| 二、 | 該校第一階段招生就學申請書寄送截止日期為110年3月17日（星期三），以郵戳為憑。招生對象為國民中學階段學生（含國小六年級畢業生），經原籍學校審定，因家庭功能失常、學習動機低落所引發之中輟情形，並經評估學生現況不適合學校學習，需於中介教育學校輔導者。 |
| 三、 | 申請審核通過之學生，民和國中慈輝分校將另行通知辦理後續轉介事宜。 |
