| 一、 | 依據臺南市政府教育局115年5月22日南市教安(二)字第1150740231A號函辦理。 |
| 二、 | 本案申請資訊說明如下，餘請參考旨揭作業要點： |
| (一) | 申請資格：國民中學階段學生，因家庭遭遇變故而中途輟學，經追蹤輔導返校而無法適應就學環境，或因家庭功能失調，以及因列為中低、低收入戶而有中輟之虞，經監護人或法定代理人同意接受輔導就讀。 |
| (二) | 申請時間：即日起至115年6月12日（星期五）前提出。 |
| (三) | 申請審核通過之學生，永仁高中將另行通知辦理後續轉介事宜。 |
