說明：
一、依據楊梅國中106年3月15日梅國秀分輔字第1060000870號函辦理。
二、106學年度如欲申請個案轉介至楊梅國中秀才分校，請依簡章規範內容及文件
格式，依期程提出申請轉介，學年初轉介於106年4月5日至106年5月9日止；學
年中轉介於每年7月至次年3月底間辦理。
三、學年中轉介個案經社政(教育)單位具備專業執照社工師或心理師評估者，由學
生在學學籍學校準備申請資料併同評估報告，向本局提出申請；無評估報告者，
由學生在學學籍學校準備申請資料，向該校秀才分校提出申請。
